What Is Alpha 2
Macroglobulin (A2M)?
Alpha-2-Macroglobulin, often called A2M, is a large plasma protein found naturally in the blood. It is produced primarily by the liver and plays an important role in the body’s response to inflammation.
In orthopedic care, A2M is valued for its ability to bind to and help neutralize certain proteins that contribute to pain, inflammation, and joint tissue breakdown. Because of this, concentrated A2M may be used as part of a non-surgical treatment approach for select patients with arthritis-related pain or inflammatory orthopedic conditions.
How Do A2M Injections Target Pain at the Molecular Level?
A2M injections begin with a simple blood draw performed under sterile conditions. The blood is then placed into a centrifuge, where it is spun at high speed to separate the plasma from the cellular components.
At this stage, the process may look similar to standard PRP. However, concentrated autologous A2M requires an additional and important step. The plasma is passed through a specialized filtration system using a pump machine, allowing the A2M-rich portion to be concentrated.
The final solution contains a high concentration of Alpha-2-Macroglobulin. Once prepared, it can be carefully injected into the targeted area to help address inflammatory proteins that may contribute to pain and joint breakdown.

Patient Evaluation and Selection
The first step is identifying the true source of your pain. Before recommending A2M injections, Dr. Cuéllar will perform a detailed history and physical exam and usually review imaging to confirm the diagnosis.
A2M may not be appropriate for every condition. For example, an unstable spine may need stabilization, while a large disc herniation pressing on a nerve or the spinal cord may require a surgical option such as disc replacement. Preparing for an A2M Injection
Before your A2M injection, you may be asked to stop anti-inflammatory medications, such as Advil, Motrin, or ibuprofen, for up to 5 days. Certain supplements, including turmeric, ginger, and fish oil, may also need to be paused, especially if the injection is being performed near the epidural space of the spine.
Patients are typically encouraged to drink plenty of fluids before the procedure. If anesthesia will be used, Dr. Cuéllar’s team will provide specific instructions on when to stop eating or drinking, which is often 8 to 12 hours beforehand.
A2M Injection Side Effects
A2M injections are generally well tolerated, but like any injection-based treatment, side effects are possible. Most are mild and temporary.
Possible side effects may include:
Temporary pain, swelling, or bruising at the injection site
Mild inflammation as the body responds to treatment
Infection, though this risk is low when proper sterile technique is used
Rare allergic reaction to materials used during the concentration process
